As the the lawful parent and/or guardian I consent to the participation of my child(ren) in all activities conducted by Mumz and Munchkinz.
I assume all risk of injury or harm to the child(ren) associated with participation in Mumz and Munchkinz Play Session activities and adventures and agree to release, indemnify, defend, and forever discharge owner and employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of injury, loss or damage to the child(ren), howsoever caused, arising by reason of or during the child(ren)'s participation.
